Unite North Dakota is a community data exchange of health and social service providers, sponsored by CyncHealth. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Organizations offering billable services will enter into a participation agreement to data-share with the health information exchange platform before joining the network. There may be a cost associated with data-sharing, but no additional charges to join Unite North Dakota. The network is supported by a North Dakota-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite North Dakota is part of a groundbreaking multi-state initiative, including Unite Iowa, Unite Kansas, Unite Minnesota, Unite Missouri, Unite Nebraska, and Unite South Dakota, sponsored by CyncHealth. This work ensures that communities and individuals can seamlessly access much-needed resources and services, such as food, employment, transportation, and housing, through collaboration with local community-based organizations. Partners are connected through a unified network, powered by Unite Us, to support their clients across state lines.
Unite North Dakota is currently serving Burleigh, Emmons, Grant, Kidder, McLean, Mercer, Morton, Oliver, Sheridan, and Sioux counties.
